The Catholic Foundation of Oklahoma scholarship application will be available after January 1, 2013 for the 2013-2014 academic year.  Although the specific program and application will not be finalized until later this year, below is a list of requirements that have been used for the last several years. The Scholarship Committee anticipates that the guidelines will be followed again this coming year:

All forms and required information must be submitted to the Catholic Foundation office NO LATER THAN 5:00 PM on Tuesday, April 2, 2013 in order to be considered. Applications are considered complete only if the following items are received by the stated deadline:

1. Completed scholarship application (form enclosed)

-If under the age of 18, application must be signed by a parent or guardian.

-Recent photo (optional)

2. Two Recommendation Forms – (1) Recommendation from Pastor or Catholic College Chaplain, and (2) Recommendation from School Dean/Advisor or Principal

  • If you are receiving an application via the Catholic Foundation website or e-mail, please note:
    • In addition to the application, you will print a RECOMMENDATION FROM PASTOR/CATHOLIC COLLEGE CHAPLAIN and a RECOMMENDATION FROM SCHOOL DEAN/ADVISOR or PRINCIPAL.  There are two pages for each Recommendation Form.
    • Each Recommendation Form has a page with “APPLICANT FILL OUT THIS SIDE” written on the top of the form, which is Page 2 (back side) of the form.
      • The one that says PARISH BACKGROUND should be attached to the Pastor Recommendation Form.
      • The one that says EDUCATIONAL INFORMATION - COLLEGE should be attached to the Principal/Dean Recommendation.  You can either copy them on the same piece of paper or staple them together.
    • It is suggested that you provide your Pastor or Principal with a stamped envelope addressed to the Catholic Foundation as a courtesy. This will also help ensure that all required parts of the application are returned to the Catholic Foundation in a timely manner.

3.  Most recent official high school or college transcript

4.    A copy of their official ACT/SAT or MCAT scores

5.    A copy of your parent’s 2012 Federal Income Tax return.

  • If you are no longer financially dependent on your parents, your Federal Income Tax Return and a statement regarding independence will be required.
  • If your parents are divorced, a tax return from both parents will be required.  The non-custodial parent may mail their return directly to the Catholic Foundation office.
  • If your income is from farming, we will require a copy of both 2011 & 2012 returns.
    • We only need the first few pages of your tax return (adjusted gross income must be included).  If you feel we need more information than is on the summary sheets, please feel free to provide that also.

During the 2012-2013 academic years, The Catholic Foundation awarded over 70 scholarships. However, the number of scholarships varies from year to year and is usually determined at the time the scholarships are awarded.

It is the applicant’s responsibility to make sure all materials arrive on or before 5:00 PM on Tuesday, April 2, 2013.
