   Have the following details ready, or you can download and complete this form to email directly to Mary Smith at msmith@cfook.org: 
    
   • Number of shares
      
   • Name of the stock or mutual fund
      
   • Designation (e.g., CFO Ministry Fund, St. John Vianney Endowment for Seminarian Education, your parish or school)
      
   • Your broker or transfer agent's contact information 
      
   • Approximate date of the gift transfer
